2. Provide X-Rays
Bring recent X-rays (less than one year old) or we'll request them. If needed, we'll schedule new X-rays for you.
3. Complete Paperwork
Save time by completing all necessary intake paperwork online before you arrive for your first visit.

Payment & Insurance
Success Legacy Chiropractic & Wellness is a self-pay practice. We do not bill or accept health insurance directly. This allows us to provide personalized care without being limited by insurance requirements.
If you have out-of-network benefits, we can provide you with the appropriate documentation, including a Superbill, that you may submit to your insurance company for potential reimbursement. Reimbursement is determined by your individual insurance plan.
