A prepaid maintenance option consisting of 12 Focused visits to be used within 12 months.

Monthly Memberships

Membership care is not a substitute for a new evaluation when a patient develops a new injury, significant change in symptoms, or condition requiring active care.

The most comprehensive membership for patients who want full-body care and regular soft-tissue/recovery support.

Best for highly active patients who consistently use multiple services.
Package vs. Membership
Additional Services
Individual services — including e-stim, IASTM/scraping, cupping, and dry needling — are available to complement your care. Membership holders receive 10% off additional services. Pricing for individual services can be discussed at your consultation so care is tailored to your needs and budget.
Payment & Office Policies
- Success Legacy Chiropractic & Wellness is a self-pay practice.
- Insurance is not billed directly. Patients may request a superbill to submit to their insurance carrier for possible out-of-network reimbursement.
- Reimbursement is determined by the patient's insurance plan and is not guaranteed.
- Treatment recommendations are based on clinical need; purchasing a package or membership does not guarantee a particular outcome.
- A new complaint, new injury, or significant change in condition may require a new evaluation or a return to active-care visits.
- Package expiration, membership billing, cancellation, rollover, refund, transfer, and no-show rules should be reviewed in the applicable patient agreement.
